BURSARY APPLICATION FORM
INSTRUCTIONS
To download the Bursary Application Form, click on the link below. You can then either:
a) print the form; fill it out by hand; and give it personally to the Director of One Future at a Time
OR
b) print the form; fill it out by hand; and scan it into an email to: [email protected]
OR
c) fill out the form on the computer (To fill out the form online, your computer must have Adobe Reader.); "save as" to the
computer's desktop; and attach to an email to: [email protected].
NOTE: To keep your personal information secure, do not leave your Bursary Application form on a public computer after you
have submitted it. Save it to your memory stick if you wish, but delete it from the public computer.
